Community Link Worker
Community Link Workers are able to assist with all aspects of social care including advice on finances, social opportunities and general advice
Community Link Workers provide a person centred approach in response to the needs and interests of GP practice patients supporting them to identify issues that affect their ability to live well. The CLWs will work collaboratively with patients supporting them through decision making processes to identify issues that they would like to address and help them to overcome barriers to improving these.
Link workers work in partnership with a number of statutory organisations and voluntary services, as well as internal resources. In addition, they utilise social inclusion and community mapping approaches to develop links with other agencies within the community and create opportunities for the patients they are supporting.
