Chronic Disease Monitoring

If you suffer from a long term health condition such as asthma, diabetes, COPD, hypertension or heart failure, you are invited to attend an annual health check on the month of your birthday.  This will involve some blood tests and, if required, a follow up with the Practice Nurse for health advice.
